Cagrilintide Spray is supplied as a pre-dissolved, sterile-filtered solution in amber glass bottles fitted with a metered atomizer, at 10 mg per unit. Amylin-family peptides are notoriously prone to self-association and fibril formation, which is the practical reason a buffered pre-dissolved format outperforms a powder here: reconstituting an aggregation-prone peptide by hand is where most material is lost. Headspace is nitrogen-purged before capping, and no reconstitution is required.

Store Cagrilintide Spray refrigerated at 2°C to 8°C (36°F to 46°F), upright and protected from light, where sealed units remain within specification for up to 24 months. Once the atomizer is first actuated, use the unit within 30 days. Do not freeze and avoid vigorous agitation: amylin-family peptides nucleate beta-sheet fibrils under mechanical and thermal stress, and once fibrillation starts it is not reversible by filtration or warming. Brief transit excursions to 15°C to 25°C (59°F to 77°F) are tolerated.

Cagrilintide is a long-acting amylin analog, and the receptor it targets is unusual because it is not a single gene product. Cao and colleagues published the structural basis for this in 2022. Receptor activation couples through Gs to adenylyl cyclase, raising cAMP. Cagrilintide also retains activity at the calcitonin receptor itself. A fatty acid side chain drives albumin binding for extended duration, and the design distinguishes it from GLP-1 agonists such as Semaglutide, which engage an entirely different receptor family.
